Please read carefully the following exclusions for the FREE LASIK ASSESSMENT :

• The assessment is free for only one time, any second assessment will be chargeable as a full consultation

• This is not a general consultation for general eye issues of retina, eye pressure, dry eyes.

• Lasik assessment does not apply to patients under 18 or over 59 years of age , or patients over 40 who have difficulty in reading only (please book a general consultation)

• Lasik assessment does not apply to patients who have done Lasik or any kind of eye surgery before (kindly book a general consultation)

• Lasik assessment is not a consultation for prescribing or changing glasses

• LASIK assessment does not apply to patients who are diagnosed with cataract or who have previously done cataract surgery 

• The free preliminary assessment that will determine eligibility to laser vision correction. You will acknowledge that if you request your reports or tests done they will be chargeable.
